In consideration of my child’s participation in CK Gymnastics classes, events, camp and activities, I agree to be bound by the following:
- Eligibility: I agree to comply with the rules of CK Gymnastics.
- Readiness: I/my child will only participate in those classes, events, competitions, and activities for which I believe I/he/she is physically and psychologically prepared. Prior to participation, I/he/she will have practiced my/their exercises and will perform only those exercises that I/they have accomplished to the degree of confidence necessary to assure that I/they can perform them myself/themselves and without injury.
- Medical and Liability Release: I understand that CK Gymnastics, its coaches, and other staff members will not take responsibility for injuries sustained by any student during the gymnastics, or cheerleading tumbling, or in the course of any exhibition, competition, or clinic in which he/she may participate or while traveling to or from the event. I’m fully aware that participation in these activities involves motion, rotation, and height in a unique environment, and such carries with it a responsible assumption of risk. I have been warned that catastrophic injury or paralysis can result from these activities. I consent to have my child or children participate in the programs offered by CK Gymnastics. I, my executors, or other representatives waive and release all rights and claims for damages that I or my child have against CK Gymnastics or its representatives. I give permission for the staff at CK Gymnastics to contact an ambulance or such other medical care providers as may be deemed responsible and necessary in the event of injury to my child. Parents should make their children aware of the possibility of injury and encourage their children to follow all safety rules and the teacher’s instructions. I have read and will abide by the policies of CK Gymnastics.
- Photo Release: I give CK Gymnastics permission to use photos/or videos of my child or children for advertising, website, or other media outlets.
- Payment agreement: Monthly tuition is due on the 20th of every month and the tuition will be charged automatically unless you pay in full for the session. Tuition is considered late as of the 25th of each month & you will be assessed a late payment fee of $10.00.
- Registration agreement: The programs at CK Gymnastics is three-month commitment and that your child’s spot is reserved for them therefore, we do not prorate nor refund tuition for any reason including injuries. An annual $40.00 non-refundable registration fee(January - December) is due when registering for your child’s first class at CK Gymnastics.
- Refund agreement: There is no refund if any gymnast drops their class without a health issue and it is required to submit a letter from health care providers. ( $15 processing fee will be charged)
- No make-up classes are offered for personal absents (Except hospitalization)
As a legal parent or guardian of this athlete, I hereby verify by my signature below that I fully understand and accept each of the above conditions for permitting my child to participate in classes, events, competitions, and activities conducted by CK Gymnastics.
