Party Waiver

CK Gymnastics party Waiver and Agreement

Children under the age of 5 must be accompanied by an adult at all times.

In consideration of my/my child’s participation in classes, open gym, events, and activities at CK Gymnastics, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that gymnastics and related activities involve inherent risks, including but not limited to falls, collisions, physical injury, paralysis, or death. I voluntarily assume all risks associated with my/my child’s participation.

2. Readiness and Compliance

I confirm that I/my child is physically and mentally able to participate and will only attempt skills appropriate to their ability level. I agree to follow all safety rules and staff instructions.

3. Release of Liability and Medical Authorization

I release and hold harmless CK Gymnastics, its owners, coaches, staff, and representatives from any and all claims or liability arising from injury, illness, or loss occurring during participation or travel to/from activities.

I authorize CK Gymnastics staff to obtain emergency medical care for my child if necessary.

4. Photo and Video Release

I grant permission for CK Gymnastics to use photos or videos of my child for promotional, website, or media purposes.

5. COVID-19 Acknowledgment

I understand that COVID-19 is contagious and that participation may increase the risk of exposure. I agree not to attend if experiencing symptoms. I voluntarily assume all risks related to COVID-19 exposure and release CK Gymnastics from any related claims.

By signing below, I confirm that I am the parent or legal guardian of the participant, have read and understood this agreement, and agree to all terms stated above.
